If you want to play in multiplayer, you need to create an account in-game. Most achievements only unlock for the host, and some pop after you exit a map. "Medic!" does NOT work in MP at all, only with bots.

Workshop map that can help for some achievements
To give yourself $10k for the start of a round, click on "Start", "Player", scroll down to "Clothing" click on the arrow, click on "bot1" and change name to "imamillionare". If done correctly you get a "cheater" notification. You will have to do this each time you join a new map.
You will need 85 ZKW points to unlock 5 challenge maps and the GiGaGirlGannon. So maybe do that first before buying anything else so you do not forget and spent everything on other stuff. The weapon can be found under "Player" and "Weaponry". Once unlocked you can find it in the fridge randomly.
Playtime only counts while on a map, but you can just pause the game and tab out. Do NOT cap your FPS below 60 since the game will run slower and therefore it would take longer to reach 15 hours.

Longer Version

The rest of the ZKW points I would use to unlock the "XPL-Bullets!" for 100 and then upgrade the weapons of your choice. Upgrading the Shotgun makes sense since that will be your only gun available for the Challenge mode. I would put all available points in reload speed.
Some help for playing Carnage on normal:
- With the money cheat just always go for the best gun you can find. You have 3 slots available and start with the Katana and a pistol.
- Once your ammo is gone you will need to rebuy a gun for that slot.
- To open doors you will need to be at a specific round, then use money to unlock it or wait until they open on their own (not recommended). 1 door per map(?) needs a key.
- For the level 30 grind at the end i would recommend to play whatever map you like until round 25-35 and then die to restart from Round 1. I preferred the first 3 maps.
You can try getting the "Medic!" achievement on the workshop map:
Join a "Carnage" game, click on "Custom Maps" and choose the workshop map. Get yourself a good gun, go to the jumpad on the left and press "e" to jump into the Zombies. The bot will hopefully follow you and die sooner or later. Then try to kill a few to have some time to revive.
Or just play a normal round and wait until the Bot dies, it took until round 12 though when i did it this way.

Even longer: Order of how i would do stuff

You can immediately start with the grind to level 30, do the challenges of surviving 4 minutes on all maps or familiarize yourself with the game first and do some achievements on the workshop map.
It seems playing on workshop maps does not grant XP, so you will need to actually play the game for a few hours.
Looking back i would start with just playing the game first and do some achievements in the meantime, then do the challenges, go back to grinding and maybe clean up whatever you missed on the workshop map.
Click on "Carnage", choose "Normal" difficulty (apparently it gives more XP compared to easy and only here can you unlock Bots by going to the cellphone icon.
Once unlocked, you only need to use Bots for the "Medic!" achievement. To add a Bot just click on the "+" before you choose a mode. Otherwise playing with a Bot is not recommended, since XP is not shared.
Cleanup:
Incase you didnt do the following yet and do not want to do it on a standart map go back to the Workshop map:
- I can NOT confirm that "I Want To Be Free!" & "Tank Dispencer!" are possible on workshop maps, so you might need to do this on a standart map. But with the money cheat it is not hard to survive, to open all doors and getting to around level 25-30 to kill all tanks. And there is always the option to add Bots or play with others. Even randoms might join your game.
- Reach round 20 in Carnage mode and get all weapon/kill achievements.
- Buy from the fridge until it dissappears and reappears 3 times and until you found 5 meat.
- "Taxidermy!" (kill the Bear in Round 15 without getting your money stolen) unlocks after exiting the map.
- Once you are done with everything except getting 10k kills, getting to level 30 and playing for 15 hours go back to playing Carnage. If the last achievement is to play for 15 hours you can just idle on any map, just pause the game and tab out.

Easy/Fast Wins
If playing on official servers, use console command "ChangeTeam" to switch to the winning side before match ends.

If self-hosting, set ResourcesTeamInitial=100 under [hrl_game.HRL_GameUPDvSH].
Play on cm_port2 and games end after the first kill. Recommend turning off bots and playing with dummy/friend.
Use AdminLogin <Password> & AdminRestartMap to instantly restart map.

How to host here and here. Requires port-forwarding - use Steam Server Browser to see which.

Walk north and enter the house.
Follow the corridor past the staircase and enter the living room.
Pick up the Broken Flashlight from the table with Q.
Pick up the Bulb from the couch.
Turn on the flashlight with F.
Backtrack to the corridor and head upstairs.
Once upstairs, turn around and follow the corridor to some shelves.
Click the Diary, then inspect it with E and read it with R.
Turn left and head through the double door.
Turn left again and follow the linear corridor to a shelf.
Interact with the shelf with E and head inside.
Next section is timed. Pause if you need time to read.
Interact with your sister...
Facing the bed, turn left and head to the corner of the room to find a report.
Interact with the report, rotate it, then read it.
Pick up the hammer from the nightstand left of the bed and use it on the door by the report.
Make your way downstairs and out the house.

Office Elevator

Talk to everyone before going down the elevator. There's a total of 39 interactable NPCs:
- Your boss (the person you spawn next to).
- 33 people in the main room (one of them is always moving).
- 2 in the bathroom (one is hidden inside one of the restrooms).
- 3 in the kitchen.
- The two NPCs from the locked room don't count.

- Once the Blabby achievement unlocks, move to the elevator and ride it down.
- Pick up the box and ride the elevator back up.

Warning: There's a missable here (Tongue-tied). Don't get detected by the monster! If you do, you'll have to re-do it in a second playthrough.

- Pick up the lantern and go to the kitchen. Pick up the key located below the microwave (screenshot: https://imgur.com/d9Vj9DS).
- Open the door to the main room. You'll find a power panel on the first row to the right side.

- This will turn the power on. You'll find a yellow keycard next to the windows on the ground (where the janitor used to stay). Pick it up.
- Return where you came from and just to the right of that door entrance, enter the Restrict room with your yellow keycard.
- You'll find a toolbox here. The password is 295. Pick up the screwdriver and use it on screws from the vent entrance.
- Go inside the bathrooms and pick a pink keycard from the second restroom on our side.
- Pick up a key from the trash. Use it on the bathroom door to exit.
- Go to the locked room (where the two non-interactable NPCs used to be), and use the pink keycard on it. Read the note.
- Insert 428 at the boss's door.
- Pick up the gun from the desk and kill the monster with it. If you haven't gotten detected, the achievement will unlock after it dies.
- Go to the elevator to finish the game.

Easiest way to unlock Full Tilt is to let yourself be detected by the monster in the main room, and move between the offices (you'll have to go for it in an extra playthrough).

This guide does the Requiem Aeternam achievement in a second playthrough, but there's no reason why you can't do it in the first. The achievement is for defeating all 10 base game bosses without healing via a Bile Flasks.

Do everything for "Witness of The Miracle" before fighting the final boss in Ending A. Do not start NG+ if you haven't unlocked Witness of The Miracle.

Crossing Souls/Rebirth Savescum
Normally only one of these achievements can be unlocked per playthrough. If not savescumming, unlock Rebirth first and do Crossing Souls in the second playthrough after unlocking Bronze Medal.

To minimise the amount of work with savescum:

Do NOT talk to Redento in Patio of the Silent Steps after defeating the Amargura blocking the way into Mother of Mothers. If you have the Big Toe Made of Limestone, you are too late.
Do NOT talk to Lvdovico in Albero Church after talking to Cleofás. If you have the Cord of the True Burying, you are too late.
Do NOT "End Her Suffering" if you talk to Socorro.

Continue through the story (skip collectibles cleanup for now since you won't have the Nail Uprooted From Dirt) and collect the three Marks of Refuge. Return to Cleofás and offer all three Marks of Refuge to Socorro. Talk to Cleofás once more and he will move to Archcathedral Rooftops. Do NOT talk to him there.

Save and backup your game now.

Talk to Cleofás in Archcathedral Rooftops.
Talk to Redento in Patio of the Silent Steps. Unlock Crossing Souls achievement.

Restore the backup.

Talk to Redento in Patio of the Silent Steps. Finish his quest for the Nail Uprooted From Dirt.
Talk to Lvdovico in Albero Church for the Cord of the True Burying.
Talk to Cleofás in Archcathedral Rooftops.
Go back for whatever collectibles you skipped and talk to Cleofás who's in Albero Church now to unlock the Rebirth achievement.

Second Playthrough - Bronze Medal Playthrough

Can be done on NG+. This is a 3 hour speedrun through the first half of the game. It's not particularly difficult as you can skip all the backtracking of the first playthrough. Savescumming is also an option.

My setup was as follows:
Pick Penitence of the True Guilt at the start. Travelling is harder because you can't heal, but it absolutely tears through bosses. Don't rush, you have plenty of time.

Upgrades
This is what I grabbed (in order):

Our Lady of the Charred Visage: Pelican Effigy, Bead of Blue Wax, Drop of Coagulated Ink, Fire Enclosed in Enamel, Debla of the Lights and Smoking Heart of Incense (deal a couple of bars of damage with your sword first because Debla alone won't kill)

Additional Information:

All collectibles in the prologue are missable!
Backup your save files ('...\AppData\Local\PlagueProject\Saved') before confronting Varg. In case you die, close the game, replace the save files and start the game again.
The achievement 'Rolling Required Ahead' is buggy. Make sure that the save icon appears after destroying a barrel - restart the game if it doesn't appear. Also, try to destroy not more than one barrel at once.

Online mode is shut down due to GameSparks service ending September 30th, 2022. Most of the achievements don't work either. Only "Boom! Headshot!" is possible. Everything else below is for archive purposes.

Everything can be solo'd, but recommend a boosting partner for online achievements and a controller for grinding local matches. Bots auto-fill empty slots after ~50s queue time in online matches.

Win 10/100/250 online games

Local wins count towards the online wins achievements, but the achievements will only unlock after playing a proper online match.

Only possible online.

Tie a round online

A shot can still be fired during the down animation. Get both players down to their last hit, shoot one player, then they immediately shoot back.

Roadmap

First, complete the game on normal difficulty, get all collectibles and let both Hans and Blue Knees kill you. Also, take as many pictures as possible for the '500X Zoom' achievement. If you want you can go for the speedrun-related achievements. In the second playthrough, play on hard difficulty, take 40 or fewer pictures and don't get killed by Hans and Blue Knees.
