Missable achievements in order of appearance

He was never slow
After Radio Free Nivalis achievement (not missable, but highly recommended): crash the HOVA against buildings until unlocking a sidequest about repairing your vehicle. DON'T do it. Instead, head to any garage and buy the 'Plasteel Bumpers' upgrade to increase damage resistance. This will be a life-saver during later phases of the game.

No Pressure 1/3: Second mission for Huxley. Pick him up at Nexus Cluster → drive to Gallows Row → wait for Pietr Unthrink location → HEAD TO CALEDON (same area as Gallows Row) → wait for CorpSec agent call → reach the CALEDON waypoint within 42 seconds.
Save the life of the life saver
No Pressure 2/3: After rescuing survivors from quarantine. Pick up Huxley and Pashta → drive them to destination → wait for scenes → reach destination within time (240 seconds).
Neo Cupid + No pressure 3/3: Timer starts when you collect the heart. Reach destination within 130 seconds.

Outbreak: The Nightmare Chronicles

General: The achievements for killing 100 enemies, using 10 painkillers and opening 10 locked keypad doors are cumulative.

Using the strategy listed below, all achievements can be obtained in less than 30 minutes. If you don't want to buy chapter 4, you can unlock all achievements by playing/replaying chapter 1 which takes ~1.5 hours.

Complete chapter 4 for Never Stop Fighting.

Repeat these steps for Medicine Master and Keep Firing!:

Start chapter 4 and collect the grenade launcher and the grenade rounds.
Equip the grenade launcher, enter the next room and kill all 3 zombies.
Continue to the next room, kill all 3 zombies and pick up the painkillers.
In the next room, get hit once and use the painkillers. Then kill all 5 zombies.
Quit to menu and repeat.

For Problem Solver and Just Give Up Already, switch to nightmare difficulty and:

Open the first locked door in chapter 1 ten times. The code is 1234.
Get killed by the first zombie in chapter 1 four times.

XP Guide:

The best way to farm XP is using a co-op partner on the Spec Ops "Fire Mission". The one in the plane is the one grinding XP. Destroy the first two cars, then the cars hiding on the bottom right behind the mountains, and the heli - then restart - repeat. This is the fastest way to grind XP that I know about.

First Recommended Playthrough

Play the modern-take mode on easy difficulty, find all collectibles and secret rooms, unlock all missable achievements and complete the game without dying. If you die, simply go back to the main menu and press 'Continue' or load your last save (doesn't matter if it's a manual save or an autosave). By doing so, the death won't count. You can skip all puzzles.

Second Recommended Playthrough

Play the modern-take mode on daymare difficulty and complete the game in less than 4 hours. This sounds harder than it is. Even on daymare difficulty the game autosaves. Also, you can avoid most of the enemies by running past them and you can skip all puzzles. You'll probably complete the game in ~2 hours.

Cleanup

Start the classic 90s mode and play until you can use the pistol. Now, do 100 quick reloads and 100 slow reloads. More information can be found here:

Grapple Gear

Two glitches come in handy for making some of the difficult levels more manageable:

Slide boosting

On certain sloped surfaces, if you mash the LCTRL key, you will build up immense speeds. This is especially useful for the gold time in Armada.

Death boosting

If you swing into a death barrier without releasing LMB, your grapple will stay in place. If far away, this can build up immense speed as you're pulled towards it upon respawning.

Note that some levels have changed slightly since this guide was created. Use the following depot for an identical build:

Only the first 100 levels are present in this depot, and some achievements might not unlock. However, once you switch back to the current version, any achievements missed should pop retroactively.

The remaining 125 levels are currently missing a guide, but many have near-identical solutions to each other. The symmetry levels can be cheesed by making cuts on the opposite side of the target shape; as long as your cursor doesn't directly slice through anything, the slice won't count, even if a shape on the other side is cut.

Tutorial Speedruns
Dash Level in less than 45 seconds: See 7:03 to 7:48 in video.
Wallrun Level in less than 45 seconds: See 30:52 to 31:49 in video.
Grapple Level in less than 30 seconds: See 33:44 to 34:17 in video.
EMP Level in less than 65 seconds: See 40:33 to 41:37 in video.
All tutorial levels in less than 3m15s without dying/restarting: Routing is the same, but timing is 10 seconds more lenient cumulatively, meaning you have some more room for error than in the individual level runs. See 42:51 to 46:24 in video for full run, if needed.

"No Shades Required"
Go to "Random" and type in the seed "Evil Maroon Shield" to launch a level that automatically unlocks the achievement.

Leaderboard achievements
For the leaderboard achievements, which rely on servers (shutting down at an unknown date) you need to:

be in the top 20% of times on the leaderboard for a daily, weekly, and monthly challenge all at the same time.
reach rank 1 in any daily, weekly, or monthly challenge.

Reaching rank 1 on a challenge isn't difficult if you do it on a daily, especially when there is little competition. Top 20%, on the other hand, is much trickier given that the monthly challenge becomes harder to gain a competitive time on as the month goes on due to more players going for the achievement at once.

Tip for "cup" challenges: If you die or otherwise slip up on a level, you can return to the main menu in order to save level progress but avoid penalties. This way, you can effectively retry each level in the cup as many times as you'd like, until you're confident that your time will be good enough in the long run to gain a top 20% time.

Softlocks and glitches: There are a couple of issues that will probably arise when attempting the leaderboard achievements:
1. Cup challenges not saving progress
To solve this issue, you have to get a new personal best on the level the cup is stuck on in order to proceed.
2. Grapples/collisions not working properly
Restart the level from main menu until it works. If the issue is later on the level, you can predict if it will work on a given attempt through earlier pieces of the level. For example, if a grapple near the end of a level won't work properly, you might notice that collisions are missing on a platform at the beginning; the two are linked. If the platform's collisions suddenly work, then the grapple later on will, too. You won't always have a "tell," however, so you might need to just blindly retry until it works.

All other achievements are either self-explanatory or should unlock naturally along the way of the achievements listed above.
