Nearest I can tell, this is a fault in the calculations of weight and thrust power ratios in various directions not being sufficiently and accurately calculated and displayed on the stats screen.
Even if you have full white mobile stats, it can sometimes still not be enough to get your ship to function correctly due to how directional thrusters work. The entire thrusting power of all your thrusters may be "calculated" to have enough to yaw, but it doesn't seem to accurately take into account that not all thrusters are supposed to be firing at the same time in order to yaw your ship. So when it runs the live calculations, it determines that the ship would move better up or sideways by simply strafing in that direction, rather than turning, because the "strafe" thrust comes out more effective to move your ship that way... (which also hints towards the error having an interaction with controls, as captains are perfectly capable of pitching/yawing with a ship that has adequate stats).
In most cases it wouldn't be a problem, but it seems like if you design your ship just right, the placing of gyros and thrusters confuses the engine causing this glitch/error.
The only "solution" i've found is to either design your ship differently, or find a way to make it lighter.
--And yes, i am well versed in understanding the center of mass and location of thrusters and how they play their role in the ships mobility.
