SteamVR

Index Headset Display Disconnected issue
I've had this problem before, it started 11/26/24. I went through steam support which wasn't much help. I had tried securing my cables, updating drivers, trying different ports, Enabling direct mode, Uninstalling SteamVR, and etc. I even updated BIOS and used USBDeview, which didn't help. So figured a cable was bad so got a new one and a few days later it worked. Tried playing again last night only for the same message to pop up. "Headset Display Disconnected. Please make sure your video cables are securely connected. The headset's display was not found." I've already tried everything above again, with no success. I'd rather not buy another cable, to test the theory on the cable. The only thing that I think can connect the two issues is when I tried to play helldivers 2, my monitors went black and after restarting my pc, which was the only was to fix it. It said I needed directx11 or 12 to play the game, which I've checked I do have. This is what the Main System Report says:

SteamVR Version 2.8.8 (1733790165)
Steam: Public (Public)
Tracking: No Driver
OS: Windows 11 (10.0.0.26100)
Direct Mode Graphics: Unknown ()

Displays - Extended Display
Display 0: \\.\DISPLAY1 1920x1080 @ (0, 0) Microsoft Basic Display Adapter Primary
Monitor 0: \\.\DISPLAY1\Monitor0 AOC 2752H

It has gotten to the point where I assume my PC has gotten to old. I got most of the parts, 6 years ago (give or take)

OS Name Microsoft Windows 11 Home
System Manufacturer Micro-Star International Co., Ltd.
System Model MS-7B84
BaseBoard Manufacturer Micro-Star International Co., Ltd.
BaseBoard Product B450M PRO-M2 MAX (MS-7B84)

So finally if anyone has any suggestions or anything helpful to say please let me know.

Sounds like it's having trouble connecting over the GPU...

Prior causes/fixes of SteamVR Error 200, -203, 204, 208, 210, 215:
1) AMD GPU error 200, change bitrate ( https://steamcommunity.com/app/353380/discussions/11/4038102936180242093/ )
2) HMD plugged into motherboard instead of GPU
3) GPU drivers. Do a clean uninstall and install of the latest,
4) Use a different port on the GPU,
5) Plug in only 1 monitor, try different refresh rates on it.
6) Bad adapters on the video cable, loose cable, pet chewed it.
7) Use regedit to change window's Timeout Detection and Recovery (TdrLevel) of the GPU driver ( https://steamcommunity.com/app/250820/discussions/2/4692279782565391419/?tscn=1729147381 )
