Irl time to game time
What is the time difference between the two, can someone tell me how long 1 hour in game is?

It's variable, not fixed. When you are in cities time in the game runs slower, but when out on a motorway it passes by slightly quicker to simulate the idea of driving a long distance.

Outside cities the time scale is 1:19 (I think) and in cities it is 1:3.

If you do not drive into a city, 1 in-game hour is roughly 3 minutes of real life playtime.

If you are driving then the rule of thumb is that 10 hours of driving equates to about 1 hour of playtime IRL.

My RoT is subtly different from Wolfgang's - five minutes of real world time per hour of in game driving - but of the same order of magnitude. If your journey is mostly motorway, where you can sustain or better the speed limit, then you can achieve as quick as three minutes per hour. Conversely back roads, especially those with numerous scenery towns and villages, border crossings and city driving can push the figure upwards towards ten minutes per hour (or more, potentially, but city areas in ETS2 aren't big enough that you are likely to be driving for twenty minutes at 1:3 scale).
