As for most difficult, probably the legendary deathclaw? I mean it needs some preparation since circle-strafing is not an option right off the bat. (On the other hand if you can kill the deathclaw guarding the cave entrance without alerting all others, the whole area can be done with enough mines and good luring.)
Or maybe Elijah if you didn't disable the turrets with robotic expert perk near the vault.
Although many people say they have problems with Ulysses too, for some reason.
With the perks you can get in Old World Blues. Cazadors become Freeside thug tier..
You can get posion immunity and 30% Damage boost against them. There is also a few perks you can get on top of that in the vanilla game.
I find Nightstalkers more deadly than Cazadors. High damage and knockdown. The NightStalkers in Old World Blues are especially annoying.
What about Rawr?
The legendary deathclaw is a pain to kill as he is with a bunch of outher deathclaw's in a cave.
The only reason people have problems with Ulysses, is because if you don't kill him quick enough, Marked men start pouring in and the mixture of the two can be an issue. I know had trouble with him on my first playthrough of the DLC. Ulysses also hits pretty hard with that ♥♥♥♥♥♥♥ staff. But then again, I just played through Lonesome Road again and was able to beat the ♥♥♥♥ out of Ulysses single handedly with a ♥♥♥♥♥♥♥ supersledge so....who knows. Super Slam for the win!

And Specimen 73.
They all have varying statistics but all have increadibly high health pools and/or DPS. Specimen 73 is almost impossibly fast.
And as Lord Popo said Ulysses can be a total pain if you cant kill him fast enough before Marked Men swarm you to death. But his boss battle isnt that hard if you can kill him before the room is flooded with other hostile NPCs. NV has a decent amount quite difficult bosses unlike Fallout 3 which is a cake walk once you get the Winterized Power Armor and the Tesla Cannon. It basically trivializes all the enemies in game except the high level mutants in Point Lookout.
But the giant robo-scorpion? First time I played, I just let the training robots out, and they killed it with losing only one of them. The record so far was one time when I had to activate the turrets before it died.
fallout nv: legendary deathclaw, is it even possible to survive a hit from that thing?
But serious with vanilla....i'd have to go with....idk actually i never had a hard enemy encounter since i never had trouble....only time's i'd die was when i fell.....because cliffs are the hardest enemy for me :<
