Either a bug of the mod, or it's having compatible issues with another one.

I dont' have anything else that messes with faces.

Purple means missing texture. Either you installed it incorrectly or you didn't turn on archive invalidation.

Installed with NMM and archive invalidation is active. I'm trying a sperate mod that just affects teeth. It doesn't say it had to overwrite anything.

Try toggling off on an archiveinvalidation.

Getting the other teeth mod fixed it.
Try bLoadFaceGenHeadEGTFiles=1 in your Fallout.ini and FalloutPrefs.ini file, as per the mod description

I had this problem, but I realized that I had accidentally deleted the file: "FCO - OHSB NPC Edits.esp"

When I put this file back into the steam\steamapps\common\fallout new vegas\data folder, the mouth textures returned. I guess the mod author made a separate esp file for configuration reasons. If anyone is having their game crash upon loading, I suggest deleting esp files such as: "FCO - Russel.esp" which came with FCO, despite me not choosing it during the installation process. If there is an esp file that requires/ uses another mod that you do not have, you must delete it yourself. A common culprit is "TTW versions" of mods. ... [OFF TOPIC]: For instance, I downloaded a mod called NevadaSkies, but it kept crashing when I enabled it. I went into the data folder, and sure enough, there was a file called "NevadaSkies - TTW Edition.esp". I deleted it and, voila; it started working again. TTW is short for Tale of Two Wastelands. If you don't have it, then delete all esp files that say TTW to fix broken mods.

TL:DR: missing esp files (which causes missing textures) is a lot better than having excess/unsupported esp files (which causes the game to crash upon starting the program).

You can also just disable the .esp files that you don’t have the files for in the launcher or a mod organizer. A mod organizer will also tell you if you’re missing files required by an .esp file.

This Reddit Comment thread helped me out:
https://www.reddit.com/r/FalloutMods/comments/3qwpc9/fallout_character_overhaul_30_released/

Workaround Solution: Copy Fallout Character Overhaul 3.0-54460-3-0.rar/Fallout Character Overhaul 3.0\TeethDirty\textures\characters\mouth\mouthhumanmerged.dds to C:\Program Files (x86)\Steam\SteamApps\common\Fallout New Vegas\Data\textures\characters\mouth

In my case, I copied the "mouthhumanmerged.dds" file into
C:\ ... \Mod Organizermods\Fallout Character Overhaul 3.0-54460-3-0\textures\characters\mouth
instead of directly into the New Vegas Data folder and it worked fine :)

I came from the future just to say you saved me from a week-long headache trying to fix that.

Thank you from the bottom of my heart.
