In the GECK - slightly less impossible.
With improved head meshes - somewhat doable.
With "beauty mods" - yuck.
Overall verdict - don't bother. Seems like this is the approach Obsidian took as well.
Case in point: Goul strippers.
Things have improved quite a bit over the last 200 years. Business suits have obviously made a comeback. I imagine that cosmetics have as well.
It's pretty awesome that all options result in an equally horrible mess of a character. I like to go with loud facial hair and a pink mohawk, because then I can pretend I'm a Fist of the north star henchman.
You can make a female character that is just as attractive as any male character - ie, not very.
If you want some pin-up or anime waifu ♥♥♥♥♥♥♥♥ - then look at the mods.
That's why you are already dead.
Yeah, that's exactly what I don't want but that seems to be the only options nexusmod is offering --> Girls with pink/blue or whatever hair and way too small faces.
There even was one guy who made a pretty looking character in GECK but he refused to share it because he didn't want others to play with his character....ehm yeah...creep.

That's a default face, for the record.

They always have that bizarre imploded nose and blownout cheekbones.
It's why I always tell people who want to put a female character in a mod to just copy and edit Trudy (or another character of an approapriate race). It's a lot easier than trying to save the damned souls the geck dredges up.
These are the real default faces of the four races: https://dl.dropboxusercontent.com/u/50671837/img/default_faces.jpg
When you create a new NPC, it defaults to the caucasian race and uses the default face for the caucasian race. If you change race, the editor doesn't swap over to the default face for the new race, it preserves the old values. The same happens if you change gender. Click on the reset face button and the default geometry is loaded.
If you have all the addons loaded as well, a new NPC defaults to the MarkedMenGhoul race, which is why even your screenshot of the caucasian woman looks messed up.
