How do you play, and survive, in Hardcore mode
I know the basics, you have to eat and drink, sleep, etc. but what makes this a new play style?

Well, as the name implies, it is harder. It's not just the fact you have to eat, drink and sleep. Crippled limbs can only be healed by using a doctors bag or visiting a doctor, so getting hit can really sting. Ammo also has weight, meaning you can't carry thousands of bullets for different ammo types. Fast travel is also very limited, as you can only travel as far as your hunger and thirst meters will allow.

It makes you a lot more cautious than you would on the normal difficulty, and choose combat more carefully.

Well… on paper. The hardcore needs increase quite slowly, you need to drink about twice a day, sleep once in two days, and one simple steak lasts for more than a day. No wonder lots of immersion or balance mods increase hardcore needs rates by 1.5 to 2.
The biggest issue will be the radiation if you can't find a reliable pure water source (or use a bottling mod) and the weight for water and mostly ammo if you are the shoot-on-sight type.

No, no. I say that it's not that "hardcore" for my taste but I'm a somewhat old cRPG gamer who uses all the difficulty-enhancing mods under the sun for this game.
I only said that Hardcore mode is not as frightening as most new players think it is (or how big the intro text there makes it to be). More than likely the biggest issue you'll have is the limited ammo space, especially if you are fond of the Anti-Materiel rifle or the big guns, the ammo for those is particularly heavy.

Think it as like the "need to eat" thing that was introduced in GTA San Andreas. It was rarely a hassle but the fact that it was there was enough to keep you on your toes.

no one has mentioned it yet, but stimpacks dont heal you instantly either, they go slowly over time about 10 seconds, same for some other meds too like radaway. this alone can make firefights much more difficult. And waiting and sleep dont regen health either, only doctors or stimpacks and other healing items.
so if i break every bone in my body, i just cant sleep in a bed and wake up fully healed

Sleeping does regen health, even 1 hour of sleep is enough to fill your health bar.
Limb condition also recovers to full when sleeping, assuming you didn't break your limbs.

well actually, it is only beds that you own or rent that do regen your health when sleeping in them, until you get the roughin perk which makes any bed as if it was an owned bed. one other thing is that your companions can die permenently while in hardcore mode
that companion perma=death thing seems kind of dumb, right
