Possible to have Companion Permadeath outside of Hardcore mode?
Is it possible to have companions die permanently outside of Hardcore mode? Any modifications like that?

Just leave em in the middle of nowhere :D
But if you want another companion just put them to a place you don't go to often. I never go to lucky 38. I always leave them there. I mostly stick to Boone and Rex :P

I think the idea here is that he wants to be punished for not taking care of his followers, not just leaving them somewhere to collect dust and never help him out in battle. He wants an added risk factor to the game without playing it in Hardcore mode. Notice that he never said he wanted to change his companions.

There is no mod like that but you can use the Fallout wiki to look for your companion IDs and use the console to set them non-essential. That will make them mortal. Though I highly advise some modded companion AI for that since they tend to walk right into a war zone and get killed.

Yes, and it is pretty easy to do yourself with the console:
- make sure you have your follower in sight
- open the console (~ key)
- clicking on your follower should give you its Base-ID, Boone for example = 00092bd2
- enter setessential 00092bd2 0 (this is for Boone)
- rinse and repeat for other followers
