What does karma do exactly?
Does it affect your reputation in a specific area or what?
I don't notice a difference whenever I lose karma.
Like when I crucified Benny, I lost karma but the legion liked me the same.
And I stole from Novac, but I'm still idolized.

Karma doesn't really "do" anything. It's just there to show you how morally good, or evil you are.

Do the NPC's realize it? If not I can just loot away.

No, NPCS only react to your faction repuation. You can just loot away if you want, obviously avoid getting caught.

It does affect your relationship with Cass. If you do a number of bad things with her around, she'll eventually leave for good.

Pretty much nothing, and with getting +100 for each evil person you kill, it's almost impossible to end up as anything else but a goody two shoes. It's pretty much been replaced by the faction system.

In this game, pretty much nothing. Its a holdover from FO3. There's still a couple of tiny places where it matters slightly - like with Cass - but mostly ignore it, faction reps are what's important.

Ultimately it's just a concept of you and the universe knowing you've done something good or bad and no one else. My advice: nothing happened until you get caught! ( ͡° ͜ʖ ͡°)

Very little.

So basically don't do it with anyone watching? Because I'm with the Boomers and I stole some combat armor from Pearl and now the Boomers hate me, luckily I saved right before that.

Karma Checks are minor.

theres only for the level 50 perk.

theres only for the game ending.

a few times I think it allows you to say certain dialog options, but yeah its mostly not used.
