its strange.
Realistic aiming is something I highly recommend. I've got mine set up to penalize my aim when my character is too hungry, thirsty or tired as well as a base level wobble that points in the Guns skill goes towards reducing. Pretty nice mod and works really well with those early level encounters where you're more fighting to survive rather than being an avatar of death.
I'd also recommend removing mouse acceleration as well, a quick google will find the lines you need to drop in your fallout.ini file.
The feeling when you can't hit a human from five meters if you shoot from the hip. I love that mod. The enemies still hit you more than they should, but it's still one of my favourite difficulty/realism mods. Would highly recommend it along with Realistic Reloading. (ANd Realistic Weapon Names, though that is purely cosmetic, but doesn't conflict with anything.)
