No, it's 100% likely that it's caused by mods. Just because it didn't happen right away doesn't mean that the didn't scripts break, which tend to happen with script heavy mods like project Nevada being mixed with other mods.
In my 8 years of playing the game, never once have I had this happen, or hear about anything like this unless it was caused by mods.
1. Your mod load order,
2. Try a new save file.
3. Check your mods nexus pages to see if they do edit the players water.
The correct answer is the least helpful?
I think we do need a load order.

You would already know the answers to both these posts if you had bothered to READ mine properly. I quite clearly wrote in the original post of this thread that I drank water to lower the meter to zero, waited for LITERALLY 15 seconds, looked at my pip-boy hydration tab, and it was raised up to 100ish already.
The ones marked as off are technically still mods I'm using, they're just in the Bashed Patch.
Not saying it's not possible to run a ton of mods with no problems, because people do it, but the same mods on different pc's can act differently. What works for me might cause you bugs.
And if your running a lot of mods, then something strange happens, it's almost guaranteed to be mod related.
