Iron Sights is not accurate?
When using the mysterious magnum and other pistols its almost like it is zero'ed in for 500m . Any further than about 20m i have to aim lower sometimes up to a whole inch below the head. Anyone else have or had this issue?

It varies from weapon to weapon. They are a bit off, usually, yes. Sniper scopes are accurate though, unlike in FO3, where they were really off.
Shots sometimes stray off wildly even at 100 weapon skill and when crouching. Even the most accurate weapons suffer from this.

Even at 100% weapon condition this can still happen.

It's been like that since Fallout 3
Tho it's not as bad as in Fo3

If it's happening at 100 weapon skill and crouching, it's because you don't meet the strength requirements for the weapon

If it's happening at 100 weapon skill and crouching, it's because you don't meet the strength requirements for the weapon

It happens with weapons that don't have STR requirement.
Even at 10 STR it still happens.

So it's not that.

Also Fo3 doesn't have STR requirements for wielding weapons and that also happens there.

Fallout NV has RPG bullet bending as a (mostly) intentional stylistic choice since it is a sequel to turn based games where your chance of hitting a target depends entirely on the numbers in the game and not the player's skill, it's a weird cross between real FPS and classic RPG that makes some sense but can also be a pain. Obviously, it can be very easily fixed with mods if it bothers you.
Not important and not really related but S.T.A.L.K.E.R Shadow of Chernobyl has exact same thing with bullets having random chance to just stray off regardless of your aim or even if gun is pinpoint accurate otherwise it still has that chance to fire in random direction instead where you aim.

Thats another game i know that has this odd RNG gun spread mechanic (or "bullet bending" as you called it). And that one is not really an RPG.

I gave up trying to use ironsights, because they just...don't work. Nothing shooting-related works properly in this engine. Ironsights are not aligned, bullets have mind of their own, and half the time you don't even get usable ironsights. It's either covering half your screen, or lacks any king of reference. Caravan shotgun just has a screw that is quite literally placed randomly. Not aligned vertically. 10mm pistol...there was an attempt, but it's so thick that it might as well not be there. There are very few guns that actually give a definite point of where to shoot.
I just turned off ironsights in options, and I can hit things now. Not to mention I've found my inner zen with melee character now. I have fire axe to chop, grenade launcher to go boom, and 12 gauge shotty to shoot fire with dragon's breath ammo. No need to aim :D

Every time I make a new character I try to take out the frag mines in primm with the caravan shotgun and every time i miss constantly because the sights are useless
Yes, iron-sights are off for longer distances. Disabling iron-sights and using the on-screen crosshair helps a lot, but is less visually appealing.

Figured this out when using Paciencia with guns at 100, crouching and on steady. Missed when the ironsights were dead on target. Hit when ironsights was disabled and ADS using the on-screen crosshair.

On an accurate gun with 100 skill you should not notice random bullet deviation, especially if crouching.
