Also, some enemies have such ridiculous perception stats that they see through cloaking. For example, every boss creature in OWB can see you even if you have cloak and silent running (that disables audio checks).
you shimmer as you move. also the bullets aren't invisible.
For a simpfied explanation, not accounting for numbers, perception etc. (oh, and sound, as mentioned above) This sounds as good as any answer for you can see it in action.
I like stealth but it can be overdone in games and seriously break any semblance of game balance.
I think the most common reason people blow their cover though is they are moving too fast (before the perk that allows you to do so).
The biggest reason I use stealth is to steady my aim with a scoped rifle.
