When playing Afterlife cards while Poltergeist is active, it will occasionally not do damage. It seems to most commonly occur with skill cards such as Ghost Shield, but it can affect attacks as well.

It looks like if you play a skill such as Ghost Shield first, that may prevent it from triggering on any other Afterlife cards for that turn. That appears to be the most reliable way to make the bug occur.
