Fix respawn system not working for polymorphed player entity.

Perma polymorphed player can't be cessated, so the user would still be able to control the character on the respawn screen. A robust solution may be employed later.

Don't set wait_for_kill_flag_on_death during perma death.

This fixes the player not dying on game completed.

Switch to integer setting for respawn health.
To have values accurate to what the user sees in the settings.
This will reset the value of this setting for every user.

Reserve kill_player_flag for non-reversible deaths.

In turn, have a new out_of_revives to death mark the player when no revives are available.
This also fixes the bug where the user can still revive even with 0 revives at the start of the game in ML mode.
