Lowering terrain view distance fixed this issue for me. Oddly enough if I restart my game it would be fine for awhile but lowering the setting seemed to fix permanently.

To get one all you gotta do is go to the island with the chickens a couple of times and you can find one. That doesn't work anymore?

When you added it, did you choose to associate it with Armored Core 6? I don't see the AC6 card on that link. If you don't associate it with a game, then it will never embed.
