It means if you play it on Twitch as streamer, your audience can vote on stuff in the game (like which card to pick).

This and which boss relic you take are actually the only things they can vote on, to be precise. And they can only vote on cards from post-combat rewards screens (or those from the colorless card event, Neow bonuses that allow you pick a card, Orrery, and Tiny House), not in shops, from potions or Discovery, etc. Be prepared to lose a lot with it.
