@Kaldaien how do i turn on DirectInput -> XInput remapping feature?
All i did was run UnX_Calibrate.exe and i put a check in the checkbox that says "Check this box if you use an XInput controller. Is there anything else?
In the advanced control panel it has an Input Management Tab -> Gamepad
The options are XInput, Direct Input, and Raw Input. Raw Input is flashing, so i'm assuming that wha't currently enabled. Not sure how to change it other than what i already said above.

BTW if you're struggling to replicate the issue, try to update your xbox controller drivers because i've tried everything i can to set it into XInput mode and i'm not sure what else to do short of buying a PS4 controller which i can't atm

PS4 controller has same issues as XBoxOne Controller, every combination triggers the Advanced Control Panel.
Are you sure you uploaded the right dxgi.dll?

If Valve didn't hijack the Guide button on Xbox controllers, I'd have a much more intuitive way of doing all this :)

Press LT + RT + Back briefly to bring the config UI up, but without putting it into gamepad input mode. Press and HOLD LT + RT + Back to go into gamepad input mode.
The menu shouldn't come up while trying to use the mod's other bindings (which are, by the way configurable). This menu binding is not configurable, it's going to take a lot of work :-\

There are a few compat settings that you may have to engage so that I can block input to the game. They are:
Xbox Placeholder (click 0 ON). Re-Hook XInput and Disable PS4 HID. That should get the game to stop being a pain in the ass :)
I found a different solution haha. I'm using the release 0.8.7.1 that's online and i have reshade in the My Mod folder, everything works, including all the key mappings. The only thing that doesn't work is the Advanced Control Panel, but that's because it's not part of that release.
I didn't know you have Third Party Mod support in 0.8.7.1 release.

Most of my mods are two or more DLLs. I try to isolate the game-specific stuff into a separate DLL that plugs into Special K. There are a handful of games where the mod is built right into Special K for performance reasons.

Third-party software of some sort.
Normal software doesn't start out with RtlUnicodeStringToIntger twice in the callstack. That's a telltale signature that something not belonging to the game is injecting itself.
Given the rest of the callstack, I'm inclined to believe it's video related. Probably video capture software such as ShadowPlay or whatever.
Have you tried completely uninstalling then reinstalling the game and seeing if it was just a problem with your install?
That's what I tried first. The problem kept happening after I reinstalled, so I believe the issue was caused by editing the sphere grid in Farplane.
Only other thing I can think of is to make a fresh game, get to the point where you can access the sphere grid, and see if you still get the crash. If you do, then something is borked with your computer. If it doesn't crash, then it's probably the edit using Farplane, as you suggested.
Try going back to an earlier save, before you used the Farplane tool. Chances are that it caused an issue. If it was the farplane tool, report the bug to the developer and attach your corrupt save file so he can try to fix the issue.

Which if I'm reading it correct, could mean there are far deeper things wrong with your computer (possibly even a virus or malware infection). I'd follow the article and as a last resort maybe even think about doing a fresh install of Windows, but that is a drastic measure if you aren't use to doing it.
