The mapsize is since beginning of ETS2 1:19

ETS2 was released over 11 years ago, with map segments being regularly added at 1:19 scale.
At that rate, It could take around 50 years to get a new map made at 1:5 scale or something like 70 years to make a 1:3 scale map. It would likely be a lot longer due to having to design a huge number of new building assets, many of which would be unique to certain countries.
I guess If they found a way to do the 1:1 scale at pace in future and make it enjoyably playable rather than tedious for more than a niche audience of 100 people, it would be using a different game engine for ETS3 or 4.
If you really want to try 1:1 scale, I believe that some modders have made 1:1 scale mods of individual cities although they may not yet be compatible with version 1.5
*Game version numbers aren't decimals, you can't just drop 0's.
These two games (ATS and ETS2) protray the essence of the locations well whilst not being very accurate to the real world. I cant comment on the accuracy of the US but in ETS2 Hull feels like Hull and Newcastle feels like Newcastle - the sights and feel of the place is there but there is not enough detail to be anything like the real place.
