It pretty much tells you that if you keep going straight you have the option for these highways in that direction.
But there are no R-3 numbers on the map and if you bare left .. there's no practical way it goes to Valencia ?

R-3 will turn into A-3 not far from Madrid. Meaning that it is very shortened due to the 1:19 scale of the map.

You will see one highway intersection with the A-3; E-901 exit (like in the game) and further north-east of it another highway intersection with the R-3 exit. Due to the short distance of them being appart (about 5 km IRL distance using the scale at Google maps) means that with the 1:19 map scale in the game they would be directly on top of each other. Basically the second intersection got left out because it would not fit in the game. And the sign is true to how it is IRL.
