Though the game is indeed from 10 years ago, the current version doesn't look that old, with the exception that the really old parts of the base map are in dire need of an update, (Austria was recently done and currently they are working on Switzerland) but they will come.
They are currently also working on moving towards DX12, but there's no time frame when that will be finished.

So yes, if the game was worth it back then and you like driving virtual trucks, it is still worth it.
However, as already was written, the game is constantly improved and there were some major technology steps. So if ETS2 was from another publisher or not subject to this extend of licensing, the current version would probably be ETS4 or ETS5 already. We got a new skybox/lighting, we got DX11 support, etc.
REWORK MAP gives worse fps, without mod the game is boring when you have problems with traffic and bugs. These are not available on older versions. New graphics make you tired.
