3D Settings -> Manage 3D settings -> Background Application Max Frame Rate
Perhaps you need to update your drivers? It was something nVidia added not too long ago in response to newer games doing this to folks.
You can set it globally for all games and even game specific.
It happens with many DX11 titles that is why it is in the global settings of nVidia's control panel and has been for at least 5 driver version updates.
Look for "Background Application Frame Rate" set it to 30 or less.
The first alt+tab, it seemed to behave, but a second, moments later, it completely stopped and went back up to 100% GPU usage.
Because it works fine on my end.
Give it a try, you may not dislike it (if it will works for you too of course).
Step 1 - Go to C:\Users\User\Documents\Euro Truck Simulator 2
Step 2 - Open the CFG file named "config.cfg" with either notepad or notepad++
Step 3 - find the line "uset t_limit_fps "0""
It should be one of the last entries in the file. In my case using notepad++ it is line 183.
Step 4 - change the "0" (zero) to whatever you would like the framerate cap to be. E.g. 60, 75, 90 etc,
That should set the maximum frame rate for ETS2 whether you are in game, or alt-tabbed.
