For the OP - as others said, the DLC add trailers, paint jobs, truck add-ons and more space to explore through new countries.

Get the game, look to see what DLCs take your fancy and then get them.
Map DLCs for me are the best and most important thing to get as it expands your game playing area hugely, If you only want to get one map DLC at a time, look for other posts which give you a preferred order to get them in, which makes it easier to "drive" to the new map area, rather than "fast travelling" to garages that you own.
There are also DLCs for trailers, tuning packs, heavy cargo, special transport, paint jobs, cabin accessories, flags etc.
All of the above are official an paid for DLCs.
Workshop content is further available to practically change the game anyway you see fit, but these can come with the penalty of them not being compatible when ETS2 is updated, so you either have to wait until the mods are updated or remove the mods or play with a vanilla profile.
I have 4 profiles, vanilla, promods, mods and a mac (yes I play on a mac occasionally).
So always have the ability to play regardless of the state of the game and mods.
Don't worry yourself too much with this at the moment, just get the game and play as is, get some DLC and later on after you have been enjoying the game maybe look into workshop mods. Certainly do not go down that route as soon as you start playing.
