However, step one on a list of 100 steps is still step one. Doesn't matter how much money you throw at it.

Agree.
I think you're right that people may not have set the sliders to the optimal sound on their individual equipment.
Furthermore, I am convinced that the sound equipment is of great importance for the sound quality.
- For example, try listening to some music played from the following media:
A mobile phone through its built-in speaker, through headset or external speaker connected to the mobile phone, try the same with a tablet / iPad, laptop.
Finally, try listening to the same music from a real stereo system with real speakers.
I don't think I'm the only one who thinks there's a big difference in the sound quality from these different media and speakers / headsets.
I think the built-in speakers in a laptop are the worst sound quality you can get out of the game, and the sound cards typically built into motherboards may not be the best sound cards you can get, either.
My own audio equipment is an ASUS Xonar U7 MKII sound card, where through an optical cable I send the signal into my Sony STR-DG720 Home theater receiver from which I get the sound from real speakers and in my opinion it sounds very realistic.
So how many trucks have you driven? Do you actually have any clue what they actually sound like?
