Now, what is the difference? The availability of torque. With more gears, you can have best torque/power all the time, as you are capable to keep engine on best revs all the time. So, 6 gear gearbox will be good, when not planning to carry heavy stuff, on flat surfaces. It would be more efficent (last gear ratio is much smaller, so car uses less fuel while cruising) then. On the other hand, if you are planning to hauling 60t over the mountains, it would be better to have even 16, or 18 gearboxes.
I'm personally using 12 automatic (I have h-shifter, but I went into automatic as a more convenient way), as it fits my needs ;) Tried 6 speed, and I though it changes gears quicker, but I'm not sure about it). However, I like the sound of changing gears, so, the more the better ;)
Dunno about semi-trailer, but I'm pretty sure that International Workstar also offers a manual 6 gear gearbox:

When I was driving manual 12, I used 7 gears anyway ;) usually it was 4-6-(switch to high gears) 8-9-10-11-12. And that was quite enough, except mountains, where splitting gears is very handy and helps to maintain speed uphill.
But most of the time I stick to 12 automatic because it's more relaxing so i don't have to focus on the gears.
But whenever i want to use a manual transmission, i found that 6 is easier to work with (with G29 gear box) because it's very close to when i drive a car in real life and i don't have to worry to split the gears. But It is very poor when climbing hills.
Using 12 in manual is also fun and challenging to drive if you own a proper wheel and paddle set like G29 or G27. Because you need to focus on the rev and the split/range.
But i prefer 12 automatic.
6 gear manual is great fun with the right amount of power/torque.

P.S don't put a car in the 3rd gear when starting to drive. I did that once because two days earlier I had been to a driving lesson in my truck, and when I start driving it I always start in the 3rd gear, so the car stalled all the time, doh!

Didn't find more gears to be useful, It's just skipping them anyway.
Managed to successfully deliver the 62t locomotive with a 400hp engine with the 6 speed gearbox, so even heavy weight is not bad. With the 12 and 18 gears, the truck spends more time on the clutch then actually pulling, that's my feeling.
But i also like the 12-speed boxes, especially for the multiple reverse gears... In ATS i exclusively use the 7 speeds, turned out to work perfectly for me :)
if there were 8-speeds in ETS2, i´d use those for sure ;)
