I'm giving up.
That indicates a memory issue.
Just ran a OCCT check, it is perfectly fine, no error detected.
That means nothing without the DxDiag report.
ignore the person asking for DxDiag (DxDiag only reports basic system information and issues with directx)
the error "Fatal D3D error (25, DXGI_ERROR_DEVICE_HUNG, 0x887a0006)" hints at driver issues or something is wrong with the hardware itself.
since you've already done a DDU and have up to date drivers and nvidia drivers typically include the most up to date DirectX drivers that are compatible with your GPU and it starts to happen again it the error could be hinting to
1 - your GPU is overheating
2 - if you've overclocked your GPU it could be causing instability
3 - your GPU could be getting hit with 100% usage which can be causing the crash, but with your settings being low to medium this is unlikely.
i'd recommend cleaning your dust filters on your PC if you haven't already. also disabling any overclocks you may have on your GPU, also you said you did stress testing on your GPU with OCCT, however if possible try stress testing with heaven benchmark, furmark or AIDA64, try to run it for about 30 minutes up to an hour, if there's any failures during them then you're likely dealing with a failing GPU. which your next step would be contacting the retailer you got it from to do a RMA
if there are no crashes during those stress tests, then it is most likely a game bug causing the issue.
also lets tackle the "memory issue" DxDiag can't diagnose a memory issue, to look for that you'd want to monitor task manager, resource monitor (slightly more advanced version of task manager), performance monitor or third party tools.

Looks like a hardware issue.
Increase virtual memory to fixed 64 GB minimum and disable XMP/EXPO in BIOS in try running the graphics card in debug mode.
Your DxDiag reports is indicating that the overclocking is currently an issue. Maybe that will address it already.
Hi, wanted to say that I was having a similar problem to the original poster. I would have an issue where I would be able to play the game completely fine, but when loading into a quest (usually if the quest being loaded is located in Wyveria , sometimes others) the load bar would reach roughly 80%, freeze. Then one of 3 things would happen.
1. My drivers would crash
2. I get disconnected from the lobby or hunt
3. It magically fixes itself and i make it into the hunt.
Number 2 lead me on a wild goose chase and lead me here. Saw アンジェル's name in so many threads lol and this one was the one that helped me perfectly. I opened my BIOS and disabled XMP. Gave it the ol' QA testing and re ran the scenarios where I would experience the issue and I'm clean. Checked out your posts in general and found your trouble shooting guide which is huge. Keep up the good work and thanks for your technical assistance for the community.

Lots of people have no crashing, then a patch comes out and all of a sudden people that have played for 100hours now are crashing, like others, every 20-60minutes. What happened? Did their PCs all magically change coincidentally the same day that a new patch came out?
Of course not. The issue is the game. Do not let these people gaslight you into thinking the problem is on your end.
This game is already renowned for horrible performance and optimisation and it causes crashing for lots of people.
Do not waste your time sending in your crash reports or attempting their "fixes". We just have to wait and hope capcom gets it together and fixes their game
Also mine NEVER crashed before today. switching controllers stops the crash for me. Its weird but try it.
Now, I can run the game on high settings without any issues.
