Selected item is switching back to Throwing Knife

Hey, I have a weird conundrum.

Sometimes when I am playing, I have the selected pouch item set to First Aid Med. For some reason, after I use the radial wheel menu to select "Optimal Health Recovery" (which is usually Mega Potion when I need it in most cases), it will switch back the selected item to Throwing Knife, sometimes.

I'm not sure what's going on, or if it's some button mispress, an option configuration, or a game bug, but maybe someone can help figure this out?
