Fatal D3D error (25, DXGI_ERROR_DEVICE_HUNG, 0x887a0006)
I searched and found a thread where someone was able to fix this in another game by doing these steps:
1.Right click on the game in your steam library and select Properties.
2. In the general tab look for launch options and type -force-d3d11

I'll give that a go, but DD2 never crashed when I played it last year, the only other game that does is WoW.
The DXGI_ERROR_DEVICE_HUNG error I am more familiar with is due to overclocked settings. I recommend checking the BIOS for XMP/EXPO and disable them, and running the graphics card in debug mode if possible, and check if that resolves the issue.

Considering you hardware I do not think you need to play with lowest settings. You should however get rid of the high res. texture pack and do tip#16 to reset your shaders.
Regarding the refresh rate you might be on the right track - as in capping the FPS can address the issue as well. Set it global to 60 FPS via the AMD software in addition to the ingame setting. Give it a try.
Funnily enough in your DxDiag report "wow.exe" was mentioned in there under the windows error reporting section. Also something about RADAR_PRE_LEAK_64 related to discord.exe, double check that discord did not re-enable their gaming Overlay, because they just updated it and they turned it back on for everyone and it was causing issues for me previously.

I don't have the high res texture pack installed currently, but I have deleted the shader cache
I've tried this before, including changing the refresh rate of the monitor itself to 60hz, didn't help, sorry I forgot to mention it in the initial post
Yeah that update pissed me off and I immediately disabled the overlay, so that's not the problem. I haven't actually had WoW crash since I did the disk cleanup stuff yesterday, and I've been running it at 200% internal resolution the whole time and that usually causes a crash within half an hour, so that might've been fixed even if Wilds wasn't.

In doubt consult AMD support regarding tuning down an overclocked GPU. Some use RivaTuner/MSI Afterburner for that. With Nvidia it is a simple check box tick.
