I use controller. Since you are on keyboard. Circle for me is whatever button you use for the charged sword attack and triangle is the other attack button. L2 for me is whatever button you use for the reticle aim thing.
U need phials for every buff and you need shield buff for phial discharge (big ass linear succession of explosions).
Hold L2 and hold circle to get shield buff when u have phials (duration of every buff including this one depends on amount of phials and if they were amped phials or not, you can tell when they are amped because they have a little fire visual over them).
Now that you have shield buff ull need to build phials again because the buff consumes them.
-Now with both phials and shield buff up, Press circle while holding L2 and while still holding L2 spam triangle+circle to do your phial discharge. If you perfect block an attack and have shield buff and phials, you will do animation much quicker and skip one of the attacks straight into phial discharge. If you have shield buff, sword buff, and axe buff you deal more dmg.
If you just want to go into axe mode rather than triangle+circle press circle and spam it for a horizontal attack that uses up some phials for extra damage or press triangel for an uppercut that i think counts as an offset attack and has similar properties. If you have axe buff you will deal more damage. You get axe buff by pressing R1 on wound.
Your sword attacks deal more damage if sword buff is up. To get sword buff you need phials, while holding L2 press and hold triangle until u see the shield fully close around your sword (a little lighting comes out) and then release triangle. This will perform a vertical cut that deals decent damage and gives you sword buff, will not consume your phials (so you can immidietly go into discharge after this if you want). You cannot get sword buff until you get shield buff.
Edit: I almost forgot, if you want to amp your phials for more damage on discharge or longer buffs you have to load them when they are already full ( i.e when they are glowing red/yellow). Essentially reload when your clip is full.
-Yellow will amp up half your phials (just like normally it wouldn't load all your phials if empty) and red will amp all of them. You load by just tapping circle rather than holding while pressing down on L2.

To charge your shield you gotta charge your phials first, you can see them on the top left corner, where you healthbar is. To charge them, just hit the monster using M1, though if you want to charge them faster, hold down M2 and then release, that hit will charge them faster.
Once the phials are red, press R + M2 to fully charge them. Though to make it faster, you can press R + M2 while also holding down M2 to charge the phials and then your shield, that way you'll have the shield ready to go.
After charging your shield, press R + M2, and then hold down M1 for a few seconds and then release, you'll charge the sword that way.
To charge your axe, there is 3 ways:
1-) You mount the monster and do a final combo by pressing R on a opened wound.
2-) Perform a perfect guard by pressing R right when monster hits you, if you see like a white sparkle, that means you did a perfect guard, so after that you can follow up with a M1 to instantly charge your axe.
3-) Perform a focus strike by pressing alt + shift on a opened wound.
Once you have all 3 charged up, you'll do more damage, though to do even more damage with your axe mode, make sure the phials are charged until they become yellow with sparkles on top, that way you'll be able to do more damage with your elemental discharge moves.
Take note that you can use axe mode without doing all of this, but you'll do less damage.
Edit: Just in case you don't know:
M1 = Left mouse button
M2 = Right mouse button
Also, I recommend you use a decoration for your charge blade called "Magazine", this will help you charge your phials much faster.
M5 a roar or something then LMB to spin up the Chainsaw.
Then alternate RMB and LMB (hold each input slightly so it knows it should keep spinning).
W/A/S/D makes them slightly move in different directions.
Focus Mode is for turning, or for popping all the Wounds Chainsaw will make (which keeps it spinning more).
Eventually, you may want to look at learning how the rest of it functions.
And be utterly disappointed at how much Perfect Guard was pushed.
