I haven't seen any times myself where I thought I hit the mark but missed, but the shot is pretty precise. If the monster moves around a lot (or even just wiggles a little), you can miss a shot showing the orange mark because the bullet has travel time.
With HBG wounds are just normal hitzones, as opposed to all the crappy hitzones the monster has the rest of the time... because the devs toughened up the skin of all monsters even more so we're basically useless unless we're hitting wounds.
Do NOT bother using Focus Shot. It is just a waste of time. Keep shooting normally in the general direction of the wounds instead.
