It does feel worse when you're mounted or have recently interacted with your seikret, at a guess the game isn't clearing buffers properly.
Also agree that the hover-release radial menu feels much too grabby compared to World, once you open it you're committed to using something whether you want to or not.
Pressing R3 to use an item is jank as all heck but might be the least awful of the two, adjusted for it being bad for hand health.
MH UI is sadly something you just accept as being some of the worst ever made and try to work around. Capcom are evidently too myopic about UI design to change, but it's unfortunate that they somehow managed to make things even worse this time around.
Also if you haven't already, make a custom radial menu for yourself and you will never use the item bar ever again trust me. Another setting to change is to set item use when you have the radial menu open to only happen when you click the right stick in, makes it so much more consistent like the first guy said and you will never accidentally use an item ever.
I don't like that customizing the radial menus seems to be kind of a chore to do, because it completely changes the feel of the entire game when you make good ones. Trust me, making your own custom radials that are organized in a way that makes sense to your brain makes everything feel so much smoother.

I wish I could help you more my friend. I agree the UI is a bit much sometimes, but the game also is a lot deeper than it seems at first glance. I truly believe this is as simple as it could be.
My only advice is to browse the settings again and spend some time reading through what everything does. If you play on KBM or Controller there are icons attached to a lot of the settings in the options menu that will indicate whether that setting is for KBM or Controller only, that should narrow down the things you need to focus on by at least half. I hope this frustration does not completely spoil your enjoyment of the game, because I believe this is one of the peaks of the series.
