they're a global orginization, but they aren't a goverment, they don't pass laws or tell people how to live their life, even in this game they make it clear they only hunt to preserve life, not for the fun of it, thats about it.
If you aren't familiar with the history, basically the "Ancient Civilisation" through extreme hubris and greed started massively culling wyvern and Elder dragons of all kinds to feed their industry and provoked a response from nature in that it turned on them wholesale and almost drove them to extinction.
MH 1 starts off many centuries later where most places are pretty much in the bronze age tech wise and the nascent hunters guild has started establishing and supporting villages close to their headquarters and enforcing a policy of living in balance with nature.
This continues through the series with periodic time jumps and the guild progressing tech and gradually spreading across the continent and then beyond.
The various regions are usually ran by local chiefs (frequently Wyverians due to the 1000 year lifespan) with guild advisors or even a local guild branch.
The guild does by and large run things throughout its territory, If people are not following guild decree's they send the Guild Knights who enforce guild law with extreme prejudice.
