How to play privately while using SOS?
Let's say I want to play with just another person and then fill the other 2 player slots with npcs, but as soon as I fire the SOS, floods of players try to join and of course the password gets disabled by the SOS, so I can only check the option to make every external player to wait for my approval before joining. But doing this, I get a nonstop log of notifications of new players trying to join mid hunt and it's pretty distracting. The private lobby doesn't seem to work that way, is there any setting to do this?

If you want to play with one other friend and have the other two slots be npcs, then you'll need to be in a private lobby and then be in a link party with your friend. Your friend (once they're in your link party) can join you through Alma by going to post/join quest -> link party quests. Once they join, you can fire your sos flare for the npcs, but you need to have your sos settings set to "support hunters only" as other posts have described. With this setup, randoms can't join you (since you're in a private lobby) and you can still have npcs along with your friend.

One important thing to note is that if you are in an "environment link" with your friend, you will no longer be able to post quests with each other. If you don't intend to farm monsters on the map, it's better to only be in the link party and nothing more.

Okay, so I believe these are all the steps that need to be taken to prevent other people from joining, this is assuming that you are hosting the quests/firing the SOS flare:

1) You and your friend join a private lobby together. This prevents people in the same lobby from joining your quests because it'll only be you two. Do not go into a squad lobby, it must be Private.
2) While in this lobby, invite your friend to join your Link Party. This auto-gives an invite for your friend to join posted quests.
3) Talk to Alma -> Post/Join Quest -> Settings, set the following:
a) "Multiplayer Settings" = "Only Support Hunters"
b) "Max Members" = 2
c) "Automatic SOS Flare" = "Off"
d) You can turn "Quest Join Settings" = "Manual Accept" for good measure.

4) When you post a quest, let your friend join and then depart together unless its a Low Rank story quest which restricts that.
5) Fire off your flare for the support hunters. With the above settings it should keep the quest private and only have Astrum unit NPCs join and assist.

Agreed.

The monsters health also scale with the number of participants, so the difficulty will not change really.

You can cancel the sos after you get the party you want.

basically this, it's such a hassle but it works.
enter a hunt with a buddy then fire the sos with manual accept. wait a good amount of time until support hunters pop up on the request list. bam! hunt ends.

But I've got a feeling OP is just gaslighting the thread at this point and doesn't even try any of the suggested methods presented.
Check your settings with Alma...

World didnt have npc helpers unless you mean the palicos of other players you might find

Talk to alma set SoS settings to NPC hutners only.

Make link party, let friend join(they don't have to be in environment link), post quest, they get auto-inv(they don't need to talk to alma to join).

Once friend joins, pop SoS to get 2 npc hunters if you want them.

^that's the only way(apart from doing it the manual way which is tedious imo) to play with 1 other human player and 2 npcs.

I co-op'ed all of the story doing exactly the above, in the exact above steps.

Once you do the side quests for the cats, it's better to just duo the quests/hunts without the npc hunters as the cats are better at that point and the monster's hp wont scale as high.
