People are still going to demand your specs. Otherwise, they can't properly help you. It could be that a recent patch introduced something that is incompatible with your current hardware, but could potentially be disabled.
Tried several other things to no avail, sucks I hope they get it sorted he was getting good 45 fps in medium settings mostly, with his 2070 and ryzen 5 2600.
With that attitude there is no reason to help you.

All all of those steps? No lol
Short of buying new hardware which isn't in the cards at the moment but is in the savings for future hopefully.
However we ran through quite a bit looking through other people's solutions, started with basics moved up to some other odd end things but Tried em. Ultimately he got it playable in windowed mode with way lower settings then he had before. But nothing really changed other then their new updates, I wonder what caused it? Hmmm well here's to hoping a future update will get him back his medium settings xD
For now it is what it is, he has a potato and we know that haha
Two things come in mind:
1. testing the game in "the wrong environment" aka the RAM allocation differs when they play either solo or in multiplayer.
2. Any special tweaks, I am thinking of tip#20 specifically and some others are reverted as the update restores vanilla files and settings.
Running it in windowed mode is definitely a good call and still great with Borderless Gaming.
I can only encourage you/your brother to give the tips a try. The more of a potato the PC is, the better it works.
This is assuming you are using Windows
Well there's new updates being pushed through Windows Update, you should check if you received said updates. If you haven't disabled it, your BIOS will be updated if you have a board from Intel 9000+ / AMD Ryzen 1000+ to fix a security flaw.
When you get that update you need to have the latest drivers for your chipset from AMD or Intel's website as part of the fix or you will lose gaming performance.
If nothing in the game changed (no game update) and you otherwise don't have any explanation, I would check that out.
Also, lastly make sure you periodically restart your PC. Restart is not the same as shutdown. Shutdown isn't equivalent to a restart if you have FAST START enabled in Windows or HYBRID shutdown (enabled by default) in this case your PC will actually go to sleep and not really shutdown, despite how it loosk.
Manually use the "Restart" option and restart your PC and retry -- in fact do this first
