Correct, but you actually end up with almost the same amount of perks at the end anyway. 25 versus 30 in FO3. And a lot of the end game (level 22+ perks) are optional choices that don't really benefit the combat of your character whatsoever (except for laser commander and slayer for energy weapons and melee respectively.) After 28, you still have 11 more perks to unlock once you hit level 50, so you'll end up taking Bloody Mess at some point.
On the other hand, you get +5% damage overall. To all enemy types. With all weapons. That's actually extremely good. I agree with jp_insomniac that this perk is a must-have (at least if you're trying to get the best character you can).
Who said it was a downside besides yourself? All I mentioned was that it would make the quest harder to complete with the optional objective of collecting the heads. And it's an insignifant amount of caps to care about when compared to the 5% increase, as you already agreed on. No need for extreme exaggeration in your post.
But we seem to be on the same page about the topic of this thread, so that's all that matters. Point being, take the perk. No impactful downside, and who doesn't like more damage?
