Desynced mouth movements while talking?

Is this just an artifact of playing the game at a higher framerate than 30? I am playing the game for the first time in years, in the past I played it on a very low end PC back in 2015, and before that on my Xbox 360. I don't remember the lip flaps being so bad. I followed Viva New Vegas to get the game to actually launch and not just instantly crash.

Keep that game on 60 fps or slightly below (clamping) and make sure your monitor runs on low Hz as well. Too big value and everything will be on a treadmill. VNV has some engine workarounds, but tricks wont solve the main issue - NVs engine is a garbage.
