Could be anything really, most likely just a way to add some variaty an flavor to the world.
Although, one thing that seems to be a common theme with most Sane Ghouls is they're usually Intelligent (not necessarily "Book Smart") or very driven.

the feral one's have had that part of their brain rotted. most likely. no science has actually been done on ghouls, things like more radiation leading to feral-ness is popular belief but likely heresy as ghouls once ghoulified are immune to radiation.
Ghouls can stay not feral for hundreds of years. In Camp Search light of the survivors only one of them wasn't feral. he likely never would turn feral. 'turning feral' is anti-ghoul racism.
you become feral when you become a ghoul. or you don't become feral.
It's every ghoul's worst fear that he or she will turn feral.
