The simple solution I did is to play just few seconds of any song in Spotify then pause, then launching the game. No more audio stuttering. This must be a bluetooth issue. The other similar music apps might also work.

As soon as I plugged in wired headphones the issue went away. Yup! It's a Bluetooth thing (in my case anyway.)
So, while the game is launched and you hear the stuttering already, in the Windows Settings > System > Sound > Volume mixer. You should see your bluetooth headphones selected as output device. Select any other available device from the drop-down, and then switch back to selecting your BT headphones. That somehow magically stopped the stuttering, audio is normal now from the game. Only thing that worked for me, hope it can help someone else.
None of the solutions described here worked with me, neither did others found from googling which mainly involved having to enabled "Disable audio enhancements" or similar tweaks after doing a Windows troubleshoot of the audio.

I will not describe the entire process of trying - the solution I found was to install the ffdshow codec and exclude Falloutnv.exe from the codec`s exclusion list.
As far as I understand, modern codec packs do not contain this codec and it must be installed separately.
I tried K-lite and CCCP codec packs with MediaPlayerClassic but they have lav codec, which does not work in FNV. And there is no option in codec`s config to force the game to use it. Atleast i did not find it. Ffdshow has this option.
I don't know what about older windows codecs, but I had this issue on Win 7 sp1 with codecs from 2015 (WMP installed by default). And this issue is only in Fallout NV, in Fallout 3 everything is ok.
BT sound broken since 2018 (problem only in one game, but some dudes use streaming ability in HDMI Audio to workaround
K-lite and CCCP - must be used carefully - Mega for example can broke bunch of games if user don't know on which versions of which codecs game depend
(I am using Bluetooth Sony XM5's. I just read upwards and people were saying about using bluetooth headphones. FXSound fixes the bluetooth audio issues)
