Fallout 3 and NV require directx9, it is not included with win 10 or 11. You can get it directly from Microsoft.

So I have directx 12....and still crashes.
Do I need to revert to a lower one for game to work?

It is fine having both Directx9 and 12 installed at once

steam games usually include the necessary extra libraries. go to the steam game folder then the Redists folder. install directx\dxsetup and vcredist_x86.exe with admin permission.

if the game still does not work, run the falloutnvlauncher.exe from the steam game folder.
sometimes a missing library or your antivirus will popup a message.

check windows event viewer and also window reliability history. might be a game crash event or system related issues.

Documents\My Games\FalloutNV with files Fallout.ini FalloutPrefs.ini RendererInfo.txt

2010 game written for 2010 hardware. try monitor=60hz and fps=60 and resolution 1920x1080. maybe game wont start on your 4k 400hz fps unlimited monitor.

My NV game suddenly stopped working today after hundreds of hours of modded play, i did a fresh install. this a new issue with the steam download or something go get the gog or epic version (paid or not) overwrite the steam files and delete galaxy files, any gog file and unins files if you used gog then verify steam cache then go to windows search and type exploit protection go to program settings then add a program from file add one at a time nv launcher.exe and nv.exe from the root directory of the game scroll down find force randomization for images mandatory aslr check override setting and turn it off, then boom it works launch with steam. no fix out everything from 2013 - 2024, i have tried every fix ever devised for this game and nothing else worked including re install windows. reg edits reinstalling corrupt dlls, every fix mod nothing else worked. now im to lazy to check but im sure steam is not downloading dlls or something for the game because turning off forced aslr doesnt fix it on its own, i still needed files from gog version to get it to start.

i tested it again with a fresh install, the steam version does not work at all without the gog game files.

on W11 OneDrive prevent Fallout 3 \ New Vegas and New Vegas PCR from launch. Disabling \ uninstal OneDrive and fix path to user profile in Registry fix problem. With Intel HD \ Arc or AMD need fixed d3d9.dll All other => nonsense.

Mandatory ASLR can be reason that the game dont launch ( at least under windows 10) you can disable it under exploit protection ( windows defender) , but this make your system also more vulnerable for certain attacks
