What does the "true iron sights" option do?
I assume that it has something to do with aiming because the phrase contains the word "sights", but I have no idea what the phrase "true iron sights" is supposed to mean. To me it just sounds like three unrelated words picked at random.

Can somebody please explain to me what it means?

If you use the 'aim' option (Rclick) you will zoom in on the sights of your weapon. In New Vegas, line up the front and rear sights and your target - its the same sight picture you'd get as if firing a real gun - instead of using the crosshairs on the screen.

In FO3 the sights on the weapon model weren't aligned with the path of the projectile.

Dunno what they mean by "true" iron sights but iron sights are pretty much anything that aren't optics. I would assume the true part refers to how accurate they are.

It means you can actually use the iron sight to aim with - unlike most games where they don't actually work right.
I don't know if you played Fallout 3, but in that game when you used ADS your screen just zoomed in, in Fallout: New Vegas when you use True Iron Sights you physically look through the iron sight on your gun, similar to what most FPS games do. If you don't use it, you use the zoom in method from F3. I recommend you use it, but it's more of a personal preference.
