One common cause of this issue is a compatibility problem between the game and your operating system. You can try running the game in compatibility mode by right-clicking on the FalloutNV.exe application file and selecting Properties, then clicking on the Compatibility tab, checking the box to run the program in compatibility mode, and choosing Windows 7 from the drop-down menu. Additionally, it may be necessary to check the box for "Run this program as an administrator" to ensure the game has the necessary permissions to run.
After you successfully opened it, you'll need to mod the game otherwise it will crash many times after reaching the Mid to Late game.
