Their one of the hostile enemies of the Big Empty/Mountain, you can't make them friendly.

Open console, click on one, "Setav aggression 0". If they immediately turn hostile again then either the game forces them to be hostile, or there are other lobotomites are around which makes him angry towards you

Try this to set them as neutral...
SetEnemy xx01229f 1b2a4 1 1
Replace xx with Old World Blues' load index. It's usually 3 or 7 depending on the load order and which dlc you have.

xx01229f is NVDLC03LobotomiteFaction, and 1b2a4 is PlayerFaction

It an individual lobotomite has already started combat with you use stopcombat on them.

From a lore perspective it doesn't make sense that lobotomites attack you but not eachother. You are a lobotomite as well when playing the DLC.
