Verify your files.

Those are all in the base game folder.

I already have and am doing it several times, but the files just keep disappearing after that

Try installing to another Steam library folder outside of Program Files.

perhaps your antivirus is quarantining those files?

perhaps run a chkdsk on the drive?

Already tried that, the quarantine history thingy doesn't show up with anything

malware? maybe do a deepscan?

those files have been sitting in my game folder for years and never disappear.

mods? mods cause all kinds of errors.

as mentioned above, uninstall the game. delete the folder from your steam library.
install fresh in another folder somewhere else.

False, as in a literal impossibility. No NV mod could even remotely cause that.

A mod manager? Potentially, but not unless you told it to. And it wouldn't keep deleting them.

It's a system issue, nothing to do with the game, mods, or Steam.

I just moved everything from steam to my D drive, that should fix it right?

That doesn't help because I already downloaded NSVE in the normal executable section thing

BEFORE you install any mods or mod managers or anything else,
try getting the game running vanilla.

if you have lots of mods, save them all in another folder.

delete the fallout nv steam folder
delete the c:\users\yourWindowsLogin\appdatal\local\falloutnv folder
go to Documents\My Games\FalloutNV
and delete Fallout.ini FalloutPrefs.ini RendererInfo.txt
if you have saves that have been modded, rename/move the Saves folder

reinstall the game. run the game FIRST and WITHOUT mods/mod managers.
are the libvorbisfile.dll, steam_api, and binkw32.dll files STILL being deleted?

I'm still deep scanning the computer and such

after reading the link in post #9 above, I am leaning towards it has something to do with nvse/nmm/mods. same missing files. so a vanilla installation then run the game should work.

I mean, at one point Vortex asked if I wanted to delete or replace some files, and I think I pressed Delete, that do anything possibly?
