0% Vats Hit Chance on everything but the head and a small percent of the torso.
On creatures, this goes to a 0% on everything. I've tried reinstalling the game, but it still happens. I suspect is might have to do with hardware or windows 11, but don't know what to do.

Possible Fix: It seems like it had to do with using integrated graphics instead of the GPU

Your character is not directly pointed at the enemy, instead it hit a wall or object instead.

What guns are you using? Remove some mods that may cause this?

V.A.T.S. targeting is bugged for certain guns - most lever-action rifles and a few pistols: it consistently shoots high, making headshots improbable or impossible. Consistently replicable by starting a new game, adding a brush gun (id: 00121148) and .45-70 ammo (id: 0013e43e), forcing Guns to 100, and shooting at Easy Pete's head from the nearby road. Bullet impacts will appear on the windowpane behind him - and, shot after shot, will consistently be high. Modifying the gun to have perfect accuracy and/or setting fVATSSpreadMult to 0.01 will reveal that the V.A.T.S. point of aim is roughly a foot high. No known fix yet. This bug also seems to occur with the Hunting Revolver.

If this doesn't answer it, don't depend on the Steam Forum, the problems I'm showing here are already possibilities that you tried. Try solving them yourself (as in deleting the Data File and reinstalling New Vegas) or wait for another comment.

Its not that it shoots high, its that in VATS shooting at humanoid limbs and most parts of creatures are listed at 0%, meaning I can't target those parts, even at relatively close range. I've tested with some of the given add on weapons, and they list 0% for hitting limbs. I've removed all my mods and reinstalled the game, deleting the related folders, but it doesn't seem to work

Are the percentages the same if you use the console to max out the appropriate weapon skill?

Yes, I've tried setting to 100, the added image shows it better.

Hmmmmmm....and that persists on a completely clean install, right? Did you do a clean install then just the basic performance/stability mods?

Right now, its just the given dlcs and the game.

The only thing I can think of is maybe some obscure engine bug?

Try installing the basics - xNVSE, Tick Fix, JIP LN, and Stewie's Tweaks to cover like 99% of the known bugs, and see if that happens to fix it.

Sorry I don't have a magic bullet here, that's an odd one.

hey, have you found a fix yet? i do have the exact same problem. it doesn't seem to be related to mods, i've spent this week trying to figure out what whas it (i was playing with the whole viva new vegas mod setup). maybe we can figure this out? i didn't have this problem on my previous win10 laptop, now that i've switched to a newer win11 laptop this bug occurs. i'm pretty sure it's hardware or os related like you said
