"I forgot to remember to forget"

As I approached Nellis to speak to them about a quest I completed (raising the bomber), I just got a notification that I've failed the quest.
I skimmed the Wiki, and I can't figure out why. I had just came from Bitter Springs, but I don't remember initiating any such quest.
Nellis also started shelling me. Are they supposed to do this?

My current companion is Veronica.

Very weird, this is boones personall quest. My guess is that he somehow died. Try finding him in Novac

I had recruited Boone, and witnessed some of his events. I wasn't aware companions were able to actually die though. Is this only when they are actively following you?

I think the last time I saw him, I sent him off to the Strip, but fast travelled before he could warp out, so he might have started wandering towards the strip by foot.

pretty sure he can die if you no longer have him as a companion. Walking on foot im not sure is possible, try finding him either in novac or the strip penthouse

I went to Novac, in the clearing near the motel, waited 12 hour until night, and my wait was interrupted by Boone, asking me "Is it time".

Now he's asking me to begin the quest, so thankfully, it's all sorted itself out. Hopefully, Veronica doesn't get herself killed.

Speaking of companions, can the Lucky 38 only accommodate so many occupants before they're sent to their default locations? I remember several occasions where I've bumped into one of them in their original location, and other times, when they haven't appeared in the suite (although on one occasion, a companion did appear/load in after a short while).

you can keep them all in the penthouse
