Difficulty ONLY affects the player.
NPC's fighting each other are always on normal.
First off, the good news: notice the different opinions and realize, there is no wrong choice, depending upon how you personally decide to use your companions.
Glass-cannon builds are for PCs, not followers and inadequate AI choices. Essentially, companions aren't as effective as I'd prefer, to rely on them as glass-cannons over the PC. Setting them as stationary ambush "turrets" would still benefit from increased damage though, which is valuable. I use mine more as mules and park them somewhere else just to provide buffs most of the time.
If I want to boost their abilities to deal damage, I just buff Charisma. Think of it this way, how many of us think CHA isn't a dump stat, so why would extra follower damage be key? It is situational for me, and only when I want to CC Hit Squads, while they kill them, to avoid infamy. So far, they kill them quickly enough and take no damage from stunned enemies. Other than that, I'd prefer they tank even one extra hit, while I one-shot their assailant... With practice, you can still get sneak criticals on distracted enemies for massive damage.
Any increase in DT helps keep them alive while Super Stimpaks heal over time in VH+HC. Which has been working out well in Vault 34, among other locations. On lower non HC modes, I just let them crumble and recover.
The devil's in the details. Remember, Companion perks can get bugged. So double check them. Which reminds me, my Veronica lost her DT buff, so I have to edit her in console.
And finally, there is a known bug, where taking the weapon upgrade for ED-E in one game, may cause him to have it automatically in subsequent games. Just something to think about, if you don't mind exploits. ;)
And now I wonder why I tried to be brief earlier. So many details omitted, oh well.
add- oh yeah, I forgot: don't forget that upgraded weapon for ED-E is short range only. If you see the colored shots not changing, then his perk got bugged, possibly needs fixed via console.
And got replaced with something called "companion suite".
So Charisma doesn't buff companions nearly a much any more, or not at all even.
Depending on the companion, they can still be quite decent against lots of low damage enemies.
Veronica with Pushy in Power Armour is nearly unstoppable against anything weaker than a fire gecko.
I've personaly seen Lily solo several Deathclaws on Very Hard + Hardcore with Oh Baby.

I'll have to try Veronica with Pushy, never done that.
I agree about Lily, I use her with Oh Baby!, vs Hit Squads. I just carry pool cues to repair it.
Oh yeah, add - I give Lily a spare Super Sledge. The reason? I think the NCR Vet. Ranger with AMR aims at her weapon. I'll use VATS to check its condition and if it is getting low, I interupt with companion wheel and remove it, before it breaks and gets thrown away.
