Radiation doesn't go down by itself, and once it reaches the end of the bar, you die. And yeah there are progressively worse detrimental effects for the stages of radiation poisoning, I believe every 200 points.
Doctors will cure it to 0 for 50 caps, or you use radaway to heal it. Radx and radiation suits (or armour with rad resistance) is what you wanna use when you go into places that give high amounts of radiation.
I don't really remember, I think they just lower stats and add or increase handicaps. Should show you on your pip boy.
Rads can be lowered/ removed by visiting a doctor or using Radaway. Rads can also be mitigated ahead of time by using rad resistant armour/ clothes or taking Rad-X.

As people said, get clothes/armor with radiation suits to help out, and stack up on rad-x/rad-away. Don't be afraid to back out from an area to go to a doctor for healing if need be. I recall a moment where I left a place after being on 999 rad
