If you're talking about the the quest "Three Card Bounty", all that happens is that the NCR First Recon unit gets transferred to Camp Forlorn Hope, and the quest 'I Don't Hurt Anymore" is unlocked.

You get a less depressive alcoholic-man slideshow ending but an advise don't spare the Russel bounty (Grafton i think he' s name, the tribe butcher) : if you spare him, you left 10000 caps and that bastard meet an unfortunate end in Mexico desert....

Personnaly i just spare the former raider in his shack near frosthill, he just wanted a new living.
